Home Based Services
Target Population: Youth age 8 to 17
Home Based Services utilize the Ecological Based Model. Services are rendered in the home and in the community to youth and their families to prevent the youth from being placed in a more restrictive environment.
Services Include:
Individual and Family Counseling
Parenting Skills Development
Family Role Clarification
Service Coordination
Community Linkage
Behavior Modification Planning
Liaison with Teachers/Schools
Crisis Intervention
Length of Service:
In-home services typically are provided to youth and their families for 3 to 6 months. Minimum 5 hours per week.
